Interagency Mobilization for Progress in Adolescent and Child Treatment

Bluegrass.org Service Coordinators exist to build partnerships that work to support and strengthen children and youth (ages birth through 18) with severe emotional disabilities and their families in their own homes and communities using the Wraparound Model and targeted case management. These services are provided in all 17 Counties of the Bluegrass service area and are organized into three major areas:

Bluegrass West—Anderson, Bourbon, Franklin, Harrison, Nicholas, Scott and Woodford Counties

Bluegrass East– Clark, Fayette, Jessamine and Powell Counties

Bluegrass South—Boyle, Estill, Garrard, Lincoln, Madison and Mercer Counties. 

Benefits to Service Coordination include: 

  Building a team of supports.
  Enhancing communication between all providers.
 
Facilitation of service team meetings.
  Assistance with referrals, resources, advocacy and education.
  Individualized and strength-based approach.

  Support for caregivers.
  Creation of a plan that is active and persistent.
  Service Coordinators available for crisis assistance, consultation, advocacy, education and coordination.
