Access - SCL & Michelle P. Waiver Case Management

Supports for Community Living (SCL) Waiver and Michelle P. Waiver Case Management are designed to meet the individual needs of children and adults who have developmental and/or intellectual disabilities and live in the community.  Those programs provide a wide variety of supports, which are detailed below. 

Participants have an individualized service plan which is developed in conjunction with a case manager or support broker, as well as other community providers.  There are two variations in which an individual can receive services:

Traditional Services Consumer Directed Services
Traditional services include Case management, Adult Day Training (ADT), Behavioral Supports, Occupational Therapy, Physical Therapy, Speech Therapy, Respite, Homemaker Services, Personal Care, Attendant Care, and Adult Day Health.  The Supports for Community Living (SCL) Waiver includes Residential Services, otherwise not provided under the Michelle P. Waiver Consumer directed utilizes the services of a support broker, who assists an individual and/or their representative to design a budget of supports to be recruited, employed, and trained by the individuals representative.  Services included under the consumer directed option are Adult Day Training (ADT), Supported Employment, Community Living Supports, Respite, Homemaker Services, Personal Care, Attendant Care, Goods and Services, and Home Adaptations.
