Access - Crisis Response/Prevention Program

Crisis Response/Prevention Program is designed to provide person-centered short-term response to individuals, who meet the programs eligibility criteria and are in an acute and immediate crisis.  Services can be accessed through a 24 hour crisis hotline.  Upon accessing services, a mobile crisis response team is dispatched to provide intensive supports in order to ensure that the individual in crisis is able to remain in their home and/or community.  In addition to crisis response, intensive case management focuses on program development and linkage to community resources designed to promote long term success and stability.

Crisis response services support individuals who are over the age of 18 with an Intellectual or Developmental Disability and are experiencing an immediate crisis.
